Web based "check-in" system

I'm looking to have built or buy a premade "check-in" system. Allow me to elaborate. I need a system that will allow users to log into a website and enter details such as name, date, problem description, etc. I then need that information put into a .pdf or word document, and a database of some sort. When the user clicks submit, I need the system to print a copy locally, and send a copy via email to my address.

Basically, I am starting up a compupter repair business, and I'm finally getting a store front. The only issue is I will be sharing the store front with another business, and I will not be there all the time. I want to make it as easy as possible for the other business's employees to check in computers. I also want the system to notify me, so they don't have to call me every time something comes in.

This seems like a pretty simple thing to do - but I really don't have the experience or the time to fool with. Does anyone know of something that is pre-existing, or is it something that one of you [H]ardocpers want to take on? I'm willing to pay of course, PM for details if you decide to go that route.

Sharepoint might work. Or just commission somebody for a custom solution.
