I am able to get my Android phone, my iPad, and my laptop to wake my desktop computer as long as I am home within my home network. I have tried a few apps and software to allow wake on WAN (wake my laptop using an internet connection from outside my home network), but none will work. All testing was done with a disabled firewall. I have a cable modem and my Win7-64 desktop is connected to the cable modem via hard wire. What am I missing?

From what I understand about this technology, the origin of the wake-on-lan magic packet must come from a device on the LAN.
From your description, it sounds as if you are not using a VPN, so it may be due to you being on a different network.

Port Forwarding is done via your router's admin page (192.168.0.1 or 192.168.1.1 generally). There should be a tab or a page specifically for port forwarding (it may be under the NAT section), in which it asks for the internal IP address the app is located on, as well as the port the app uses.
As for which port(s) need to be forwarded, that's app specific, and you'd need to try to find that out yourself.

Wake on LAN is a Layer 2 Ethernet technology. You have to be on the same Layer 2 Ethernet Broadcast Domain for it to work. You'd need to install some kind of repeater software to get it to work. Which would mean you'd need an active device on your home network.
